We are very glad you are considering Sanford Hematology Oncology for your Hemtology or Oncology care.  Here are a few important items to remember when first scheduling your appointment with us.

Please have the folllowing informatoin when you visit our clinic:

  • Patient’s legal name
  • Patient’s address and telephone number
  • Patient’s valid insurance information
  • Patient’s date of birth and Social Security number
  • List of patient's current medications
  • Patient’s referring doctor
  • Emergency contact (if desired)
  • Financially responsible person:
    • Legal name
    • Current address and telephone number

    • Social Security number   

  • In order to make an appointment please call our office at (919) 775-8183 and request an appointment.  We will schedule you as soon as possible.
  • If you presently have health insurance, you are expected to bring your current insurance card with you at the time of your visit.  Please bring a list of all of your current medications with you to your clinic appointment.
  • If lab work is performed during your visit to our clinic you will receive a separate bill from UNC Health Care  McLendon Labs.
