New Patients

We are very glad you are considering us as your primary health care office.  Here are a few important items to remember when first scheduling your appointment with us.

In order to become a new patient, you will need to acquire a UNC Health Care Medical Record number. To do this you will need to follow these instructions:

  • Contact our Customer Call Center locally at (919) 966-1234 or toll free at (866) 595-3175 between the hours of 8 a.m. – 5  p.m. Monday – Friday.
  • Our automated system has the option to select English or Spanish.
  • Select the option to obtain a New Medical Record Number (MRN).
  • If it has been a long time since you were seen at UNC Health Care and you do not remember or have your Medical Record Number (MRN), please advise the representative of this so a thorough search can be done prior to assigning a new number which may not be necessary.  Make sure that any name changes that have occurred are addressed as well.
  • Please have the following information readily available:
    • Patient’s legal name
    • Patient’s address and telephone number
    • Patient’s valid insurance information
    • Patient’s date of birth and Social Security number
    • Patient’s referring doctor
    • Emergency contact (if desired)
    • Financially responsible person:
      • Legal name
      • Current address and telephone number
      • Social Security number   

  • In order to make an appointment please call our office at (919) 806-3335 and request an appointment.  We will schedule you as soon as possible.
  • If you presently have health insurance, you are expected to bring your current insurance card with you at the time of your visit. If you are not able to bring your insurance card with you, and we cannot verify your insurance with your insurance company, we may need to re-schedule your appointment or ask you to pay $70 at the time of check-in, at least until we can confirm your insurance coverage.  Please remember that you are ultimately responsible for your bill.
  • Please bring a list of all of your current medications with you to your clinic appointment.
  • If lab work is performed during your visit to our clinic you will receive a separate bill from UNC  McLendon Labs.
